This project integrates the PCP service coordination model and e-referral processes into Melbourne and Western Health HARP initiatives, aiming to improve patient care through integration, coordination, and improved continuity of care. The project targets chronic diseases like diabetes, COPD, chronic heart disease, and aims to address wide disciplinary challenges. Key outcomes include better stakeholder partnerships, information flow, and improved client outcomes with strategic alignment to patients' needs.
