MICS Questionnaires The survey unit is a household: A person or a group of persons living and eating together HOUSEHOLD QUESTIONNAIRE Usually for a sub-sample of households QUESTIONNAIRE FOR INDIVIDUAL MEN (AGE 15-49) QUESTIONNAIRE FOR INDIVIDUAL WOMEN (AGE 15-49) QUESTIONNAIRE FOR CHILDREN UNDER-5 (AGE 0-4) Questionnaire Form for Vaccinations at Health Facilities
