Arthritis and DiabetesIn Utah FACTS: During the combined years 2003, 2005, 2007 and 2009 • 22.0 percent of Utahns reported arthritis. • Native Hawaiians/Pacific Islanders (14.0 percent) • Hispanics/Latinos (18.8 percent) • 6.4 percent of Utahns reported diabetes. • Prevalence rate of diabetes higher among special populations • Native Hawaiians/Pacific Islanders (7.0 percent) • Hispanics/Latinos (9.7 percent) • Among Utahns who reported diabetes, 50.1 percent alsoreported having arthritis.

  7. FACT: • Among Utahns who were obese during the combined years 2003, 2005, 2007 and 2009, 31.2 percent also reported having arthritis.

A Successful PartnershipReaching Pacific Islanders • National Tongan American Society (NTAS) • Utah has the largest Tongan population outside the Kingdom of Tonga • Organization with a focus exclusively on the Pacific Islander population (primarily Tongans, Samoans and Hawaiians) • NTAS has been serving Pacific Islanders since 1995

A Successful PartnershipReaching Pacific Islanders • National Tongan American Society (NTAS) • Responsible for CDSMP delivery to the Pacific Islanders of Utah • Consistent delivery and coordination of workshops, sites, leaders and participants • Successful partnership due to ability to directly access and reach the Pacific Islander population, as well as average workshop participant number of 13

A Successful PartnershipReaching Hispanics • Alliance Community Services (ACS) • Hispanics account for 10% of the population over 20 years of age, which makes it the largest minority group in Utah • Subsequently, Spanish is the second most common language in Utah • Organization with a focus exclusively on the Hispanic/Latino population • ACS has been serving Hispanics since 2002

A Successful PartnershipReaching Hispanics • Alliance Community Services (ACS) • Since 2009, ACS has been the lead agency responsible for reaching and connecting Hispanic-serving organizations with Tomando Control, and increasing delivery to these groups • Conducts Spanish Leader Trainings and Tomando Control workshops, as well as assisting in the identification of new delivery system partners
