  2. Official Graduation Dates • Boston University has three official graduation dates: • May (date changes each year) • September 25th • January 25th

Sign-out • Sign-out sheets can be downloaded from our website: http://www.bu.edu/dental/students/graduation/signout/ • Click on graduation/sign-out, (located on the menu at the left) and then again on • Postdoctoral sign-out /diplomas • Click on the appropriate department • There are separate sign-out sheets for certificates, degrees & fellowships.

Certificate of Completion • Certificates of COMPLETION are issued when a student has obtained all of the necessary signatures on the sign-out sheet before the official university graduation date, which is September 25th, 2012. • Requests for certificates of completion are available in G428 or can be printed off our website.

  6. Certificate of Completion(Example Letter) This is to certify that Dr. Tooth completed the course of study required for the CAGS/MSD/DSc/DScD/PhD and in accordance with regular procedure will receive this certificate/degree on September 25th, 2012. the next official graduation date of the University. Dr. Tooth has no further educational obligations to the University and therefore it is recommended that this student be granted all of the privileges of this certificate or degree.

  7. Certificate of Graduation • Certificates of GRADUATION are issued when a student has obtained all of the necessary signatures on the sign-out sheet and the official university graduation date has occurred. • Requests for certificates of graduation are available in G428 or can be printed off our website.

  8. License and Board Applications • Please read all materials carefully prior to submitting the application and fee. • All states/boards do not have the same requirements. Many states will not accept a “Letter of Completion.” • Some states will only take an official transcript with the certificate, degree and graduation date on it. You can receive an official transcript at any time after you sign out, but you can only be issued one with a September graduation date printed on it AFTER September 25th, 2012.

Graduation Programs 1) All University Commencement Program: “The Red Book” 2) BUGSDM Commencement Ceremony Program • Both will reflect your name and previous degrees as it appeared on the student link as of March 15th, 2012. • Your name will appear in both programs whether you are a September or January graduate, unless you have restricted your data on the student link. • Your certificate and/or degree annotated in the programs have been confirmed by your department chairs.

  10. Health Insurance • If you are currently enrolled in the AETNA student health plan, then your coverage officially ends August 31st, 2012.

  11. Are you coming to the Dental School’s Graduation? • Please notify Ms. Lindsay Corrigan in the Office of Event and Meeting Planning if you will be unable to attend the graduation ceremony. She is located in Robinson’s Bldg. 72 East Concord St., room B-341. Her e-mail address is lindsayc@bu.edu
