Event Analysis Task Force (EATF) • Membership is OIWG and OPEAS Members • Meet by conference call monthly • Select Events (EA Reports) for presentation to OPEAS • Human Error and Equipment Failure are top initiating events • Designed to capture Category 1 and 2 events that do not require EA reports but have valuable information August 2014 OPEAS Meeting

NERC Lessons Learned Overview • High AC Voltage Can Lead to Remote Terminal Unit (RTU) Failures • Circuit Breaker Modification Leads to Inadvertent Trips • Improved Contractor Oversight Needed • Generation Relaying – Underfrequency Protection Coordination • Generation Relaying – Overexcitation • Verify That Actions Called for in Operating Guides Can Be Implemented Within Required Time Frames August 2014 OPEAS Meeting

NERC Lessons Learned Overview • Loss of SCADA Due to Memory Resources Being Fully Utilized • Load-Shedding Plans for Localized Events • Loss of EMS Monitoring and Control Functionality for More Than 30 Minutes • Event Analysis Process document is out for comment • Comments due by 10/10/2014 August 2014 OPEAS Meeting
