  2. Executive Summary Jhpiego is currently implementing a process for improving the quality of health services through the implementations, measurement and reporting of performance standards. This project is called Standards-Based Management and Recognition (SBM-R) and is currently implemented in over 20 countries. Jhpiego’s vision for this innovative application is to provide a standardized tool that can accommodate data entry and reporting of performance and quality measurements. These measurements are constructed for a particular country, around clearly defined service delivery processes or a specific content area.

Project Overview • The SMB-R is a practical management approach for improving the performance and quality of health services and comprised of four basic steps: • Setting performance standards that are constructed around clearly defined service delivery processes or a specific content area • Implementing the standards in a streamlined, systematic way • Measuring progress to guide the improvement process toward these standards • Rewarding achievement of standards through recognition mechanisms • Data is collected at health service delivery points (hospitals, health centers, health posts) through forms in MS Word. The data on achievement of standards is currently housed in separate spreadsheets (MS Excel) or basic relational databases (MS Access) in each country. There is currently no standard database or application. One country office is locally developing a web application in Portuguese. • In current MS Excel and Access files: As the SBM-R instruments change over time and across countries, the denominators (# of standards observed) may change and the tables must be updated so that the percentage of standards achieved is calculated correctly according to the version of the instruments. In some but not all current SBM-R data files, the standards themselves are not listed, only the total number of standards and number of standards achieved, in order to create summary indicator tables and graphs.

Benefits of New Application • Centralized management of application but not data. • Increased reporting capabilities (currently limited by Excel and Access) • Ability to increased standardization across countries • Ability to list the text description of standards so that reports could show which standards were met/not met in addition to summary tables and graphs • Ability to summarize increasing numbers of facilities and data collection points over time. • Internal and external assessments could be supported and include side by side reporting feature

Recommended Architecture - Rationale • Common Application • Shared application with ability to meet individual countries data entry and reporting needs • Allows for rapid deployment of new standards, denominators, or reports across multiple countries • Significantly reduces overhead and maintenance • Data Isolation By Country • Shared application code, separate databases • Each country's installation will function on a unique sub-domain • The sub-domain (i.e. malawi.jhpiegosmbr.org ) will be used to determine the countries application requirements as well as determine that country's database connection and country-specific settings • Technical Considerations • Supports low bandwidth access • Ability to install local copy of database and application • Proven Architecture & Experience • Currently deployed in 17 countries for the International Training and Education Center on HIV (I-TECH), for the Training System Monitoring and Reporting Tool (TrainSMART), University of Washington Department of Global Health
