Emergency Caesarean Delivery of Preterm Baby Boy: Case Report and Management Details

DESCRIPTION

This report details the case of a 38-year-old woman, G2P1, who underwent a STAT caesarian section at 35 weeks and 2 days of gestation for fetal arrest in descent. The infant boy weighed 2300g and exhibited acrocyanosis at birth with a heart rate of 70-80. Initial resuscitation efforts increased heart rate to 120-130. Maternal history includes treated UTIs in the second and third trimesters. The baby was transferred to Level 2 care for further management, including CBC and blood cultures.

  2. Identifying Data • Live, preterm, baby boy delivered via STAT caesarian section for Arrest in Descent to a 38 year old G2P1 (1011) at 35 2/7 weeks age of gestation • BW= 2300g BL= 46 cm HC= 33 cm CC= 29 cm AC= 27 cm • MT 37 weeks AGA • AS 6,9

  3. Maternal History • 2ndtrimester • (+) UTI treated with unrecalled antibiotics for 7 days • 3rd trimester • (+) UTI treated with unrecalled antibiotics for 7 days • (+) Vaginal Spotting; admitted at VRP, given Isoxuprine

  4. Past Medical History • No previous illness • s/p Appendectomy • s/p Cystectomy Left Ovary

  5. Family History • Maternal grandparents : Diabetes

  6. Personal Social History • College undergraduate • Call Center Agent • No vices

  7. Obstetric History • G1 • 2008, blighted ovum, 11 weeks • s/p D&C • G2 – present pregnancy

  8. Upon delivery • Clear amniotic fluid, with weak cry, heart rate of 70-80, acrocyanotic, with some flexion and grimace Suctioning, Positive pressure ventilation and stimulation done • At 3 minutes: HR 120-130, with good cry, with acrocyanosis stimulation and drying continued

  9. Transferred to Level 2 • Work-up: CBCPC, Blood Culture and Sensitivity, CRP • Antibiotics started • Feedings started • BP and O2 saturations obtained • Hgt – 61  69  d/c

  10. Complete Blood Count CRP: 0.49 mg/dl Blood Culture:
