Health Disparities Supervisor • Indian Health Board of Minneapolis INDIANHEALTHBOARDOFMINNEAPOLISJOBDESCRIPTION HEALTH DISPARITIESSUPERVISOR EMPLOYEE:SUPERVISOR:UPDATED:DEPARTMENT: JOBSUMMARY This positionexiststoprovideoversightof diabetesstaff;responsibleforthequality implementationofdiabetesprogramactivities. ESSENTIALFUNCTIONS Superviseandcoordinatethedailyactivitiesofdiabetes/diabetespreventionstaff. Responsibleforstaffdevelopment,personnelmanagement,scheduling,performance reviews, trainingandothertasksassociatedwith managingpersonnel. Annuallyconduct performanceandsalaryreviewsofdiabetes/diabetespreventionstaff. Facilitatediabetes&diabetespreventionteam meetingsat least monthly. AttenddepartmentandIHBstaffmeetings,andrepresentdiabetes/diabetespreventionstaff atweeklymanagement meetings. Submit monthlydepartment reportsasrequired. DevelopandassistintheimplementationoftheSDPIDiabetesProgramswithIHSstandardsfortheprograms. ImplementationofannualIHSSDPIDiabetesProgramsgrantsreporting. Evaluate,organizeandordersupplies andeducationalmaterialsfortheSDPI Diabetes Programs. Assist in meetingregulatorystandardsregardingnutritionandhealtheducationofsuchorganizationsas JCAHO. ADDITIONAL RESPONSIBILITIES Implement theLifestyleBalanceCurriculumbyactingasaLifestyleCoachasneeded. Implement diabeteseducationthroughindividualcounseling/groupclassesasneeded. Provideappropriatesupport, communicationandinformationalresourcestopatients. ActasrepresentativeofIHB,providingSDPIDiabetesProgrammaterialsandresourcestocommunity-at-large. ProvidehealthpromotionandnutritioneducationwithinIHBandinthesurroundingcommunity(TwinCitiesarea). ParticipateinPerformanceImprovement activitiesas needed. Someeveningsandweekendswillbe required. Assumedutiesassigned by theMedicalDirectorwhenneeded. PLANNINGACTIVITIES Participateindepartmentalplanningeffortsinordertomeetdepartmentalandorganizational goalsand maintainstandardsofcare. Maintainknowledgeofcurrentandculturallyappropriatehealthcarepracticesanddiabetes care andprevention recommendations. Understandcurrentrequirementsofgrant andfundingentities. MaintainawarenessofIHSproductivityrequirements.

IMPLEMENTATIONACTIVITIES • Activeinvolvementinthecoordination,planning,implementationandreportingoftheSDPI DiabetesProgramsgrant activities. • Assistinimplementingthemeasurableobjectivesofgrantsandstandardsofcareinconcert • withotherstafftoaddressthehealthneedsofIHB’sclientpopulationandanytargeted populations. • ProvidehealthcareactivitiesinaccordancewithIHB’smissionstatement. • Assist inmeeting regular standardsofBPHC, JCAHO,andHIPAA. • KNOWLEDGE,SKILLSANDABILITIES • Professionalandpersonalintegrity. • Self-disciplinedandorganized. • Awareofculturallyappropriatehealthanddiabetes care. • KnowledgeandrespectoftheAmericanIndiancultureandotherculturallydiversepopulationsweserve. • Excellentpeopleskills. • Excellentwrittenandverbal communicationskills. • TRAININGANDEDUCATION • MinnesotalicensureasanRN,Dietitianorequivalentexperience. • 2+yearsSupervisoryexperience. • Current certificationasaDiabetesEducatororwillingnesstobecomecertifiedis desirable. • 2+yearsexperienceprovidingdiabetesandrelateddiseasehealtheducation,especiallyinan ambulatorysettingisdesired. • ValidMinnesotadriver’s license. • Computerskillsrequired.KnowledgeofAccessandExcel databasesanddesktoppublishinguseful. • Continuingeducation requirementsasneededto maintainlicenseand/or certification. • HIPAA • IHBmakesreasonableeffortstolimitaccesstoanduseofprotectedhealthinformation(PHI)by employeestotheminimumnecessaryperformanceofassigneddutiesasoutlinedinjob descriptions.ThispositionhasaccessrestrictedtoPHIneededtoprovidetreatmentandcarry out healthcareoperations inassignedclinicalarea. • PHYSICALDEMANDS • Anindividualinthispositionmaybeexposedto: • Adverseindoorandoutdoorenvironmental conditions • Bloodbornepathogens • Physicalhazards(e.g. needlepricks, etc.) • Chemicalhazards • Theworkrequiressomephysicalexertionsuchaslongperiodsofstanding,walking,recurringbending,crouching,stooping,stretching,reaching,orsimilaractivities;Theworkmayrequire specific,butcommon,physicalcharacteristicsandabilitiessuchasabove-averageagilityand dexterity. • Thisdescriptioninintendedtobeageneral statementabout this jobandis not tobeconsideredadetailedassignment.
