  1. Inhaler Technique Coaching Workshops Middlesex Pharmaceutical Group

  2. Introduction: Who are the Middlesex Pharmaceutical Group (MPG)? The MPG comprises 9 Local Pharmaceutical Committees(LPCs)representing 575 pharmacies in contract with: • 3 NCL - Barnet, Enfield and Haringey • 6 NWL- Brent, Harrow, Hillingdon, Hounslow, Ealing, ‘Hammersmith and Fulham’. • Work with key healthcare stakeholders, local councils and other healthcare organisations.

  3. MPG Partners: • NCL Children and Young People with Asthma STP board and Enfield CYP multi-agency taskforce for admission avoidance. • Enfield CCG and PHE to develop Community Pharmacy services within NCL. • Imperial Health Trust to develop antenatal and children’s Community Pharmacy services in NWL. • Royal Pharmaceutical Society to develop an Asthma Review Service in NWL.

  4. “Ask About Asthma” pledge in 2018 • Asthma patients are supposed to have an annual asthma review with GP practice. • Should include an inhaler technique check. • Patients are also offered an annualmedicine use review at their regular Community Pharmacy. • So why are so many patients still suffering or having a poor control of their asthma? • The MPG decided to direct its pledge towards training.

  5. What did the MPG do? • Set up Inhaler Technique Training Workshops (2 in NCL & 2 in NWL) • For CommunityPharmacists and their dispensary teams • Included invitations to GPs, GPtrainees and Practice Nurses • To improve the confidence and knowledge of the healthcare professionals providing asthma reviews • To improve the quality of inhaler coaching

Interactive Workshop • Outlines the National review of Asthma Deaths and explains why this training is so important • Helps with Asthma/COPD Reviews (NMS/MUR) • Helps to recognize when a patient has an inappropriate device • Recognizes when disease deterioration/ exacerbation may be due to poor control • Discusses new devices • Gives a brief overview of Personal Asthma Action Plans • Introduces the RightBreathe App- a useful tool for all professionals

  9. Pharmacist comments - post workshop • Thank you very much for organizing the InhalerTechnique Training evening. Despite practicing community pharmacy for many years, dispensing and counselling patients on inhaler use, I found the training very useful and has inspired me to attempt to coach every patient who uses inhalers on a regular basis and check and improve their technique as necessary to get the most out of the medication • It was great to be able to handle the devices, I feel more confident to show patients how to use them effectively

  10. Patient Comments - Post Workshop • I have asthma and so I showed my young children how to use their inhalers. I thought I was using them correctly but apparently not and that mustexplainwhy my children were still using a lot of the blue inhaler. The Pharmacists taught us how to use them properly and now we all feel so much better • After another visit to A/E my local Pharmacist showed me how my 6 year old daughter should be using her inhaler and spacer. We hadn’t used the spacer because it’s big but now I understand why it’s important I will. That could be why she kept needing to go back to hospital.” • My son was using his salbutamol inhaler every day so the Pharmacist explained why he should be using his brown inhaler and having this chat put my mind at rest that it was safe to use the steroid one regularly

  11. Community Pharmacy Asthma Project NW London • The LPC Inhaler Technique Training Workshop has supported the NWL Community Pharmacy Asthma Project IN NORTH WEST LONDON

IN NORTH WEST LONDON • Following award winning COPD and Glaucoma Projects, RPS in NW London developed a Project entitled: ‘Patient-centred Asthma Right Care in Community Pharmacy’ • Aim is to support patients, over 18, with more information on their condition and improve management and outcomes • Pharmacists were invited to 2 local educational events with expert speakers • 24 Pharmacists are participating in the Project • A Project pack was developed and support tools provided • Project started in May 2019 and is due to end on 31st October 2019 • The Project is being supported by Pharma companies

IN NORTH WEST LONDON Project Pack • Developed by a small team of LNW Steering Group members including Darush Attar-Zadeh, Respiratory Lead for Barnet CCG. • All participating pharmacists attended an event to receive the pack together with training on use of the tools to enable them to carry out consultations with patients. This could be during an MUR.

  14. IN NORTH WEST LONDON Pack contents: • Welcome letter • Letter to GP • Referral forms • Initial and Follow-up Questionnaires • Personal Asthma Action Plans • Flashcards for device use • 7 steps to good basic inhalation • Primary Care Respiratory Update on Peak Flow monitoring

IN NORTH WEST LONDON Pack contents: • Guidance for HCPs on inhaled Corticosteroids in Adults • Inhaled Corticosteroid Safety information for Adults (Booklet with ICS card) • Website signposting information for patients • Resources and useful information for pharmacists • Summary Record Sheet • Evaluation Form • Asthma Control Test pads • Inhaler Technique reminder stickers • SAE for returning completed questionnaires and a campaign photo

  16. IN NORTH WEST LONDON Additional items for pharmacists: • AsthmaRightCare Slide Rule • Asthma Guidelines • Mims Respiratory Devices visual guide • Asthma Posters • Stop Smoking Resources • PF meters (manual and Smart) and In-check Dial + extra mouthpieces • Placebo inhalers

  17. IN NORTH WEST LONDON Project is ongoing • Data will be collatedduring November and analysis will start as soon as possible thereafter. • Initial findings will be presented to participating pharmacists from February 2020. • Findings will be presented as Papers and Posters, with a target of at least one presentation by the autumn of 2020.

The Future for Community Pharmacy • To support PCNs by providing commissioned Inhaler Technique coaching for patients? • To support PCNs by providing commissioned Asthma Reviews in Community Pharmacy? • To support the patient by providing inhaler coaching, reviews and associated advice. • To be embedded into an Integrated Healthcare Network for a holistic approach to patient care
