In this workshop we will discuss:Why do we have THP-Plus Provider PlansWhat needs to be in a Provider Plan, and How can Provider Plans be used to improve practice
Why…… Do We HaveTHP-Plus Provider Plans? • CDSS requires all THP-Plus Providers have a THP-Plus Plan on file. • Counties are responsible for certifying that Provider Plans meet all the requirements set forth in the regulations. • Provider Plans are the foundation for the THP-Plus County’s THP-Plus Program. • Provider Plans feed into the overall County THP-Plus Plan.
Logistics of the Provider Plan • Provider Plans need to be certified by the County prior to THP-Plus services being provided. • Provider Plans need to include all required information. • Provider Plans should be reviewed and updated annually. • Provider Plans must be kept on file at the County Social Services Office.
What is in a Provider Plan? • CDSS regulations require specific items be in a Provider Plan. • Key Areas: • Cover Page • Mission Statement of the Provider • Program History/Introduction • Program Model • Population to be Served • Referral, Screening, and Selection Process • Service Components • Polices • W&I Code Section 16522.1 and 16001 • Staffing Plan • Staff Training • Outcomes and Evaluation • Certifications and Assurances • Budget
What Can Provider Plans Be Used for…. • Promote accountability: • Check and balance for the program • County and the Provider • Manage the THP-Plus Program: • Program Monitoring – the program being offered should reflect the program being provided • Fiscal Monitoring – what happens when the program expenses are reconciled at the end of the year and there are remaining funds? • Staffing Accountability – Staffing Work Plan • Program Improvement • Consisting amongst programs in a County
When Should Provider Plans Be Updated……… • Provider Plans should be updated when changes occur in the program, such as • Staffing • Budget Modifications • Program Elements (Adding or Subtracting) • Outcomes • The County THP-Plus Plan has changed
