Proof with a Single Document One of these proves both citizenship and identity: • U.S. passport • Certificate of naturalization • Certificate of citizenship

Proof of Identity for Children • Day care or nursery record • School record or report card--under 16 • Affidavits signed by a parent or guardian (under 16 only)
