  1. AB109 Process For HRC Users

  2. What is SFTP? Secure File Transfer Protocol (SFTP) is the site used to electronically transmit data in a secure manner. Access is password protected. All actions (e.g. logins, uploads, downloads and deletes) are logged and audited.

  3. Basic Workflow Process • If ROI is not signed, request will not be processed. • PRCS – Post-Release Community Supervision

  4. Receipt of Release of Information (ROI) Request Once ROI Request is received, the HRT must log the request into the data base for tracking. The request will by logged and tracked from date of receipt to date of completion.

  5. Processing the Request: The AB109 request consists of 6 Mental Health forms that will be found in the eUHR. • Signed ROI form 7385 (MedLgl) • MH 7390 AIMS (MHChrono/Misc) • MH 7447 Suicide Risk Evaluation (MHChrono/Misc) • MH 7388 MH Treatment Plan (MHNt/TxPln) • MH 7389 Mental Health Evaluation (MHChrono/Misc) • Mental Health Work Sheet • The standard turnaround time to process the request is 5-10 working days. • The signed 7385 ROI Request form will be scanned into the eUHR. • A mash up of the requested forms will be compiled and saved onto a designated share drive. • After mash up, the file will be named using the formal naming convention. • From the share drive the files will be uploaded to the SFTP site. • The files will be routed to the designated County according to the County code specified*. • After verifying upload is successful, the request will be logged as completed*. • The uploaded file will remain posted on the SFTP site for 30 days. • * See county code list • * See log

  6. Request Parameters • A signed and completed 7385 ROI request • Limited to 6 months of information • Limited to 6 approved documents • Additional information requested must be submitted through a separate signed ROI authorization form.

  7. Copy eUHRpdf files in a shared folder and then perform Mash-up in this folder. HRC User1 is allocated to work with CDC_00_to_10, and User2 is allocated CDC_11_to_20 and so on.

  10. Enter Login and Password

  11. After Mash-up: Open SFTP site and upload the final files from CDC_<>_to_<> folder to the County DMH Folder

  12. Confirm upload is successful

  13. Upload viewable to county user Once upload is complete files should appear on the “Remote Server Files and Folders” side of the screen.
