ELIGIBILITY CRITERIA • Unmarried • Incapable of self-support because of mental or physical incapacity that existed before the child’s 21st birthday or 23rd if enrolled as full-time student. • Child must be dependent on the sponsor for over one-half of his or her support or have been at time of sponsor’s death. DOCUMENTATION • Current physician’s statement (dated within 90 days of application) • If over 21 a Physician’s statement indicating the incapacitation occurred after the 21st birthday but before the 23rd. • Letter from school registrar reflecting the dependent was enrolled as a full-time student in an accredited institution of higher learning when the incapacitation occurred. HOW TO GET THE ID CARD • Once approval letter is received • Complete DD Form 1171 and go the to nearest ID Card Issuing facility with the completed DD1171 and your approval letter.
