  5. PHQIX – Who Should submit? A member of the organization that led a public health QI Initiative The submitter of the QI Initiative will be the primary contact.

  6. PHQIX – What information do I need? • Information about your QI team and partner organizations • Background on the project and implementation • Details about the evaluation of your QI Initiative. • Tools or other materials • PDF version of the submission template is on www.phqix.org
