Aims of session to highlight: • National Patient Information Pathways pilot • MCCN Health Vehicle
Background 2007 Cancer reform Strategy patients • should have: • access to high quality information • access to tumour specific core • national patient information • access to information prescriptions
Vision for information • National Information pathways • Information prescriptions • Partnership working NHS, Macmillan Cancer Support & CRUK tailored information.
Mapping Background • 168 mapped local network pathways (up to Oct 2007) • Wide variation in content and resources • Pathways largely acute focused 3 areas identified as poorly mapped at a local level • awareness & early detection • advanced & recurrent Cancer • Cancer in remission • Workshops held to identify information for the pathway
Pilot Sites • Now 10 pilot sites across country • MCCN involved in this pilot • CCO, • RLH, • Wirral Community Palliative Care Team • Whiston, • Warrington
Background • Macmillan Information bus visited the network for 13 days in 2008 • Macmillan target is 6000 contacts per year • MCCN had 1815 contacts in less than 2 weeks • Need identified for similar service • Obvious demand • High Incidence and mortality rates • Early Detection and Prevention strategy
Provision of this service? • Dedicated MCCN vehicle • 3 dedicated staff • 2 managers (1.0 wte and 1 0.5wte) • 1 admin support (1.0 wte) • Volunteers • Support of visiting specialist staff • CNS’s • Therapy radiographers • Doctors • Trusts • Primary care
Our plans • Brand new vehicle built to our exact specification
Funding • Initial 15 month project • MCCN funding for vehicle purchase • Remaining funding from NCAT • Longer term funding • Results and achievements will be reviewed and published throughout the project • Bid to commissioners for long term collective funding will be made in the 2009 commissioning round
