2. Trots stora variationer i olika populationer av oral-hygiennivå och gingivitförekomst, utvecklas svår parodontal destruktion hos bara en mindre grupp personer, ca 7-15%. 3. De flesta parodontala ytor hos patienter uppvisar ingen sjukdom trots kontinuerlig bakteriell provokation. Sjukdomsaktiviteten tycks episodisk till sin karaktär. (Andrea Mombelli ”Proceedings of the European Workshop on Mechanical Plaque Control” 1998)
Sammanfattning av forskningen ”Studies to date strongly suggest that stress, distress, and inadequate coping are important risk indicators for periodontal disease. Furthermore, it is likely that systemic diseases associated with periodontal disease such as diabetes, cardiovascular diseases, preterm delivery, and osteoporosis may share psychosocial stress as a common risk factor.” (Genco mfl. 1998)
Stress – definitions Stressors bring an organism into a state of disharmony, or threaten homeostasis. The stress-responses consist of adaptational responses counteracting the effects of the stressors. (Chrousos, JAMA 1992)
“Allostasis – the ability to achieve stability through change – is critical to survival. Through allostasis, the autonomic nervous system the hypothalamic-pituitary-adrenal (HPA) axis, and the cardiovascular, metabolic, and immune systems protect the body by responding to internal and external stress.”(Bruce McEwen 1998)
(Machiavelli i Discorsi, 1519)
The Stress System ”Activation of the stress system leads to behavioral and peripheral changes that improve the ability of the organism to adjust homeostasis and increase its chances for survival.” (Chrousos & Gold 1992)
The Stress-system ”The two principal components are the Corticotropin-releasing hormone (CRF) and Locus Ceruleus-norepinephrine/autonomic (sympathetic) systems. Their peripheral effectors are the hypothalamus-pituitary-adrenal axis (HPAA) and the limbs of the autonomic system.” (Chrousos & Gold 1992)
Equifinality ”…equifinality, the tendency towards a characteristic final state from different initial states and in different ways, based upon dynamic interaction in an open system attaining a steady state;…” (Bertalanffy: General System Theory 1969)
The Stress System The systems responsible for reproduction, growth and immunity are directly linked to the stress system These systems are inhibited at many levels during stress The dysregulation of the stress system (hyperfunction or hypofunction) has enormous consequences to human health. (Chrousos & Gold 1992)
