INTERIM OUTCOMES LONG-TERM OUTCOMES SHORT-TERM OUTCOMES STRATEGIES Impact: The health of all Kansans has improved through healthy community design Increased community knowledge about place-based strategies that support physical activity • Increased knowledge/ awareness of coalition members • Healthy communities and the link to individual health • Place-based strategies to increase physical activity (worksites, schools, childcare centers, neighborhoods, etc.) Identify, fund and support implementation of best practices that support policies, practices and environmental changes that promote physical activity. Increased community support and involvement in promoting physical fitness Community norms support healthy behaviors and environments Increased effectiveness of coalitions/communities in advocating for policies and practices designed to promote physical activity Design RFP using evidence based/best practices Distribute RFP widely to communities throughout Kansas Communicate purpose of the HCI 2 grant and best practices in supporting policies, practices, and environmental changes that promote physical activity through two-day RFP conference Select the strongest proposals through a systematic review process Provide technical assistance to grantees around coalition building/planning/ evaluation and leadership development throughout grant period. Focus of TA will be around policies and practices that facilitate physical activity Support creation of, and activities implemented by, statewide advisory group Strengthening of coalition/local leadership team: increased skills, collaboration and leadership within coalitions to advocate for place-based practices and policies that promote physical activity Sustained effective leadership around policies and practices that promote physical activity across different environments Increased champions within community among influencers/ policymakers/decision-maker Promoting Healthy Behaviors of Kansans: Policies Practices and Environmental Changes that Promote Physical Activity - Initiative Logic Model Increased # and types of key players in coalition – including unlikely suspects Increased motivation and opportunities for evidence-supported policies that support physical fitness, within community Increased physical activity among individuals in targeted communities and sites Increased ability of coalitions to form and cultivate partnerships with community stakeholders including schools, workplaces etc., that that promote physical activity Policies and/or practices are implemented in grantee communities that promote physical fitness in schools, worksites, neighborhoods, etc. Increased awareness among policy/ decision-makers regarding policies and practices that encourage environmental changes that promote physical activity. Increased resources dedicated within community to implement practices and policies that promote physical activity
