Deductive Change Order from Original Contractor (Cal Pacific) - $5, 900. • Contract to Arthulia, Inc, was awarded in the amount of $21,000.00 • Completion: May 9, 2014. • Remove and Replant damaged / improperly planted Trees - COMPLETED • Provide Recommended Fill and Guard with Stakes – COMPLETED • Repair Toilet Partition Guards /Repair Lavatory Stations – COMPLETED • Patching, Painting Surrounding all Devices– COMPLETED • Raise (24) grab bar to ADA standard (6 RR’s)–COMPLETION: 04/09/14 • Install 1 Metal Louver Installation 1’x 36”– COMPLETION: 04/09/14 • Remove and Repaint Numbers in Fire Lane – COMPLETION: 04/09/14 • Final Inspections/Punch List by AOR & IOR –SCHEDULE AFTER 04/09/14 • Re-slurring of portions of asphalt field – Cost proposal pending • The cost of additional inspections, AOR services, and closeout is also included in the Deductive Change Order.
