  8. ACC Quality Initiatives H2H – the goal of the initiative is to focus on the transition between hospital and home and thereby decrease the all-cause 30 day readmission rate in CHF and AMI patients by 20% nationally by December 2012

  9. ACC Quality Initiatives FOCUS – Formation of Optimal Cardiovascular Imaging Utilization Strategies Goal – reduce geographic variation in CV imaging and identify best practice rates Reduce inappropriate imaging by 15% in 2010 and by up to 50% by 2013

  10. What is Mission: Lifeline? A community-based national initiative to improve quality of care and outcomes in heart attack patients by improving the health care system readiness and response to ST-elevation myocardial infarction (STEMI) patients.
