  3. The role of Mentors is to: Facilitate the knowledge and skill-building capacity of waiver agents and CILs to provide effective transition services Mentor Job Responsibilities

  4. What do the Mentors do? Mentors: Provide resource information, tips and techniques for working with Medicaid and DHS Address barriers to transition and resolve them creatively Give presentations at regional and state conferences Develop tools to further educate transition coordinators Actively participate in NFT Housing Specialists meetings and Outreach Specialists meetings Submit monthly reports to MDCH tracking emerging issues and successful strategies

Mentors: • Are available for questions and guidance via telephone, email, and face-to-face visits • Help problem solve anything from resistance with NF staff to challenges in obtaining DME to relationship building • Travel to your area, agency or local nursing facilities to provide hands-on training Mentor Services

Each year, Medicaid recipients must recertify eligibility for Medicaid • DHS mails an application packet to the Medicaid beneficiary (or designated representative) approximately two months prior to the effective Medicaid month • For example, if the Medicaid effective date was June, 2010, an application packet would be mailed each year in April Medicaid Recertification

  11. When a Medicaid beneficiary is in a NF, DHS will send the application packet directly to the Medicaid beneficiary or their designated representative. • It is their responsibility to complete this packet. However, the NF Business Office Manager is available to assist if needed and often will do so. • When a person transitions out of the NF this task of having to complete an application annually may not be a priority. • To continue providing NFT supports after transitioning the person needs to have active Medicaid.   12. NFT Coordinators can play a role in recertification. When a NFT notice is submitted to MDCH via Compass for review, the Medicaid eligibility start date will be recorded. NFT Coordinators must clear the notice in Compass and verify the Medicaid status. By noting the Medicaid eligibility start date, NFT Coordinators can help remind Medicaid beneficiaries to recertify. “My Medicaid must have lapsed!”Who has heard this before?
