AIM OF THE STUDY RESULTS The study of spermatozoa with the use of novel techniques has shown an association between DNA damage and the presence of big nuclear vacuoles on the sperm head. The Intracytoplasmic Morphologically Selected Sperm Injection (IMSI) has emerged as a promising technique in the field of Assisted Reproductive Technologies allowing for the best selection of the injected spermatozoa, at least in specific infertility cases. The aim of this study is to evaluate the laboratory and clinical outcomes of IMSI-cycles and to compare these outcomes with the respective ones of the conventional intracytoplasmic sperm injection (ICSI) cycles. MATERIALS AND METHODS STATISTICAL ANALYSIS: GROUPAvs. A1: Significantly increased proportion of top quality embryos (p=0.05), Significantly increased ongoing pregnancy rate (p=0.04), Significantly decreased abortion rate (p=0.04) GROUPBvs. B1: Trends towards increased proportion of top quality embryos and ongoing pregnancy rate as well as decreased abortion rate • GROUPA • 62 subfertile couples • male teratozoospermia and/ or high sperm DNA fragmentation • IMSI • GROUPB • 35 subfertile couples • 3 or more repeated implantation failures after ICSI • IMSI • GROUPA1 • Control group: similar characteristics with group Α • ICSI • GROUPB1 • Control group: similar characteristics with groupB • ICSI • For IMSI, spermatozoa were selected under x8400 magnification using • OLYMPUS IX71 inverted microscope equipped with OCTAX cytoscreen. • For conventional ICSI, spermatozoa were selected at x400 magnification. CONCLUSIONS According to this study, IMSI seems to significantly increase the proportion of high quality embryos and the ongoing pregnancy rate as well as to significantly decrease the abortion rate in cases of teratozoospermia and/or high sperm DNA fragmentation. The above parameters showed similar trends in cases of multiple IVF failures. The use of IMSI may be beneficial when applied in selected cases of subfertile couples, underlining the importance of the paternal contribution. EMBRYO A.R.T. Unit, Athens, Greece
