  2. Spacing • 정상 발육 과정 - ugly ducking stage : 영구 견치의 맹출과 더불어 대부분 폐쇄 • 치료가 필요한 anterior spacing

  3. 발생율 • Richardson : 6세-38.6% 8세-56.8% 14세- 18.3% • Popovich & Thompson : 9세-48.8% 16세-9.3% • Lavelle : 성인에서 3-5%

  4. 원인 • 유전적인 요인, 선천적인 결손 • 악궁에 비해서 작은 치아, 과잉치, 회전치아 • 비정상적인 순소대 • 전상악골의 정중선에서 불안정한 유합 • Interseptal bone의 격벽 • 비정상적인 혀의 자세 • 거대설 • 국소 병소 ( 치주염, 낭종, 종양 )

  5. 치료 • 교정 치료 • 외과적인 수술 • 보철 치료

  6. 외과적인 수술 • Clark - frenectomy & maxillary osteotomy • Bell - interradicular & subapical osteotomy & rubber elastic • Peterson - interradicular & subapical osteotomy

  7. Frenectomy • 일차적인 요인 or 이차적인 요인 • Dewel, Fin : 먼저 설소대를 제거한 경우 제거시 생긴 반흔 조직이 정상적인 절치의 근심이동 을 방해 한다. • Bell : 교정적인 이동후 순소대를 절제하면 이때 생긴 반흔이 유지 안정에 도움을 준다. * *교정적인 이동후 소대 절제술을 시행

  8. 교정 치료 • 가철성 교정 장치 • 고정성 교정 장치 >>>현대 교정 치료에서는 어려운 문제가 아니다. Problem is “안정성”

  9. 안정성 • 기여요인 - 치료 기간 - 공극의 정도 - 연조직의 반응 : 정중 봉합부, 치은 • 확실하게 안정되게 재발을 방지하는 치료방법은 영구적인 보정 장치이다.

  10. Circumferential supracrestal fibrotomy • 재발의 방지를 위한 부가적인 술식 • Oppenheim : supracrestal gingival fiber가재발의 원인 • Edward : 회전 이동된 치아의 치주에서 많은 oxytalan fiber를 발견하였고, 이의 제거에 재발 방지의 성공 여부가 달려 있다. • Hass : 급속상악골 확대후 이개된 상악 중절치의 공극은 곧 폐쇄되는데 이곳에 oxytalan fiber가 많이 증대 된다고 하였다.

  11. Spacing Tx. • 정확한 진단 + 확실한 원인 요소의 제거 • 교정적인 치료 • 소대 절제술 + CSF + 반영구적인 보정
