  1. Tri-borough online accident/incident reporting system As of the 1st July 2013 Managers / Head Teachers guide to reviewing accident/incidents

Accident / incident reporting • To simplify accident & incident reporting the three boroughs (RBKC/LBHF/WCC) previous online reporting systems have been merged • In doing so the forms have been reduced and made easier to use • There are now only two forms; • One school specific form for capturing all school related accidents & incidents (School Incidents) • One for all non-schools accidents & incidents, including near misses, violent incidents etc. (Council Incidents)

  3. What you must report, how and when • What: • Accidents, with or without injury • Physical or verbal assaults, threats of violence or intimidating behaviour • Road accidents occurring at work/in work time or involving council vehicles • Near misses: any incident that could have caused injury or damage to property • Dangerous occurrences: e.g. failure of a boiler, release of dangerous chemical • How: • Either you or your manager / head teacher should fill out the appropriate form via the link; • Schools - https://tri-b.info-exchange.com/school • See following slides for instructions • If unsure contact the Corporate Safety Team – 0208 753 3728 / 4681 or Michael.Sopp@lbhf.gov.uk or Corporate.Safety@lbhf.gov.uk • When: • You need to report any incident as soon as is practicable to comply with the law

What happens after the form is submitted • The relevant manager / head teacher will receive an email notification • Corporate Safety will receive an email notification • Corporate Safety may investigate (based on set criteria) • If reportable under law Corporate Safety will inform the Health and Safety Executive: • deaths; • major injuries; • over-7-day injuries – where an employee or self-employed person is away from work or unable to perform their normal work duties for more than 7 consecutive days; • injuries to members of the public or people not at work (including children at school) where they are taken from the scene of an accident to hospital; • some work-related diseases; • dangerous occurrences – where something happens that does not result in an injury, but could have done; • Gas Safe registered gas fitters must also report dangerous gas fittings they find, and gas conveyors/suppliers must report some flammable gas incidents.

  5. Manager / Head Teacher role • You should receive an automated email like the one pictured below • If you do not have a password please contact corporatesafety@lbhf.gov.uk or 020 8753 4681 Click on this link to access the incident form via logging in

  6. Logging on to the system Enter your full email address and password then click ‘Log in’

  7. Updating / reviewing the incident record Unlike the previous system once you log in it will take you direct to the incident record Click on ‘Edit Incident’

  8. Head Teacher’s Review The manager / head teacher should now review the whole report and within the review section enter any actions you have taken to prevent recurrence etc. and ensure you have selected that you have reviewed the incident Ensure you have selected ‘yes’ to show you have reviewed the report After reviewing the other information on the form complete the ‘Head Teacher’s Review’ section and click ‘Save Changes’

  9. Logging on via general rather than incident specific link - https://tri-b.info-exchange.com/Default.aspx Enter your full email address and password then click ‘Log in’ This will allow you access to all of the records for the school, download PDF versions of the forms etc.
