General CCAC Info CCAC provides In Home Health Care Placement in Long Term Care Homes Information and Referral to Health and Community Services Our role is to work with clients, families and caregivers to meet individual health and community care needs Funded by the Ministry of Health and Long Term Care through the Central Local Health Integration Network

  4. Central LHIN Info Central CCAC is the most populous LHIN in Ontario Population growth rate is two times the provincial average It is estimated by 2012, over 271 000 people in the Central LHIN will be over the age of 65

  6. Central CCAC Info Central CCAC has 65 000 unique clients 26 000 clients receive service on any given day Over 22 000 clients receive PSW assistance Over 17 000 clients receive Nursing services

CREMS Process Metro Area Receives faxed referral from Toronto Central CCAC Assessed as high priority by CCAC Monthly statistics sent to TCCACC to compile and send to EMS York Region EMS calls in referral Assessed as high priority by CCAC Provides follow up call to EMS with result of each referral

  8. CREMS Statistics Have been tracked from August 2008 to present Approximately 106 CREMS referrals to date 42% were taken to hospital by EMS

  9. CREMS Statistics cont 73% of referrals were for new clients 26% of referrals were for existing clients 43% of referrals were admitted to CCAC 30% of referrals were not admitted to CCAC 11% of referrals were offered CCAC service however the clients declined

  10. Reason for Referral Falls 40% Dementia 15% Poor Coping 11%

  11. Services Provided Occupational Therapy 27% Personal Support Worker 25% Nursing 18% Case Manager follow up 17% Physiotherapy 8% Social Work 8% Long Term Care Home 8% Dietician 3%
