  2. Back to Main Page LTI Notification LTI No 01 2012 7th January Accident Location: Rig 1 at Nimr WHAT HAPPENED: Dalma Energy Assistant Driller broke his upper arm while climbing up the cellar pit. His right foot slipped causing his hand to get trapped on the rung. • RESULTS: • Broken upper arm. • 42 Work Days Lost. IP slipped while climbing up the cellar pit. He was wearing safety boots. Lighting was adequate

Back to Main Page LTI Notification LTI No 02 2012 7th January Accident Location: 3KM towards Nimr R/A WHAT HAPPENED: Abraj Energy, Ofsat contractor, working for Petrogas Rima driver with Operator was travelling to Nimr from Rima (Abraj Rig 101) when their vehicle hit a camel at about 2200 hours. The operator broke his thigh bone. The driver escaped with only first aid injuries. RTA • RESULTS: • Broken upper leg. • 42 Work Days Lost.

  4. Back to Main Page LTI Notification LTI No 03 2012 20th January Accident Location: Rig-46 - Nimr 873 WHAT HAPPENED: Dalma Enargy Junior Mechanic was raising a mud boat using hydraulic jacks. While he was pushing the mechanical support into position his finger got trapped between the two surfaces causing crash finger tip injuries, which was later amputated at Salalah Hospital. • RESULTS: • Finger Injury. • 28 Work Days Lost.

Back to Main Page LTI Notification LTI No 04 2012 8th February Accident Location: Rig-1 - Nimr 878 WHAT HAPPENED: Dalma Energy Floorman was guiding the Cavins air operated slips using a winch towards the rotary table on the rig floor in preparing for commencement of POOH (Pool Out Of Hole). He broke his finger when it got trapped between the Farr tong beam and the Calvin slips.  Calvin Slips Farr Tong Beam • RESULTS: • Broken Finger. • 21 Work Days Lost.

Back to Main Page LTI Notification LTI No 05 2012 2th February Accident Location: Hoist 31 - Lekhwair WHAT HAPPENED: MBPS Derrick man was helping the installation of the escape line pole in the monkey board of the hoist. His finger got caught between the pin and the steel floor edge causing crush injury. • RESULTS: • Finger Injury. • 14 Work Days Lost.

Back to Main Page LTI Notification LTI No 06 2012 13th February Accident Location: Hoist 31 – Nizwa Imty Junction WHAT HAPPENED: OTO driver, working under the Bahwan DHL contract was transporting pipes from Muscat to Haima. Injured his back At the Nizwa – Imty junction (23:30) when his vehicle left the road and travelled approximately 40 meters where it struck the compound wall of a residence. RTA • RESULTS: • Back Injury. • 42 Work Days Lost.

Back to Main Page LTI Notification LTI No 07 2012 14th March Accident Location: Wireline Unit (L662) - Lekhwair WHAT HAPPENED: MB Petroleum Wire-line Assistant was alighting from the Wire-line unit via a set of steps when he lost his footing on the second step from the top and fell forward onto the ground and broke his wrist. • RESULTS: • Broken Wrist. • 28 Work Days Lost.

Back to Main Page LTI Notification LTI No 08 2012 16th March Accident Location: Hoist 4 – Well Ufuq 13 Lekhwair WHAT HAPPENED: MB Petroleum Floorman was bleeding down pressure from the well through a back pressure manifold when a piece of tubing used as a bleed down line swung and struck his arm due to incorrectly secured. He suffered a double fractures, abrasions and puncture wounds to his arm. • RESULTS: • Broken and wound punctured Arm. • 28 Work Days Lost.

  10. Back to Main Page Fatal LTI Notification LTI No 09 2012 20th March Accident Location: PDO Kauther Depletion Compression Project. WHAT HAPPENED: On Tuesday the 20th March at approximately 17:20 hrs, a lorry belonging to Al-Hassan Engineering Company working as a Subcontractor of Petrofac E&C Oman LLC on the PDO Kauther Depletion Compression Project was parked close to the Kauther Gas Plant fence whilst it was being loaded with scaffolding which required removal from site. After loading was complete the driver began to reverse the lorry when seconds later he felt an impact.  He stopped the vehicle to investigate and realized that he had struck and severely injured a pedestrian who was declared dead on arrival at Adam Hospital. WHAT HAPPENED: ATE civil helper broke his leg while aligning a new welded flow line on sleepers. It unexpectedly rolled back from the sleeper and hit his leg.  • RESULTS: • Broken Leg. • 42 Work Days Lost.

Back to Main Page LTI Notification LTI No 11 2012 2nd April Accident Location: Rig 58 Hazar 13, Bahja WHAT HAPPENED: Dalma Energy Floorman was standing on the rig floor cover plate slipping drilling lines. The plate moved causing him to fall into the bottom box beam. He broke his jaw bone. • RESULTS: • Broken Jaw. • 42 Work Days Lost.

Back to Main Page LTI Notification LTI No 12 2012 3rd April Accident Location: Rig 79 Fahud WHAT HAPPENED: Sea and Land Chief Mechanic was checking and recharging the Mud Pump pulsation dampener when he lost his balance and fell. He stroke his head on the pumps discharge line and broke his skull. • RESULTS: • Broken Skull and internal bleeding. • 28 Work Days Lost.

Back to Main Page LTI Notification LTI No 13 2012 11th April Accident Location: ATE Marmul Material Yard WHAT HAPPENED: Hamood Al Rashidi LLC helper, subcontractor of  Amran Est subcontracted to ATE was in the process of loading the casing pipes using the hiab crane. He was standing between the bundles of pipes waiting to hoop up the lifting belts when the pipe bundle slipped and hit and broke his leg. • RESULTS: • Broken Leg. • 42 Work Days Lost.

Back to Main Page LTI Notification LTI No 14 2012 4th May Accident Location: Saih Nihaida WT-8 WHAT HAPPENED: Trainee Operator working for Fahud Oilfield Services (Well Test Unit-8) was walking near brine/water tanks at shift change when he tripped over a flexible hose and fell to the ground. He broke his thumb. • RESULTS: • Broken Thumb. • 42 Work Days Lost.

His broke his thumb when it got trapped between the beam and the edge of the  cement pad ditch. Pinch point • RESULTS: • Broken Thumb. • 14 Work Days Lost.

Back to Main Page LTI Notification LTI No 16 2012 9th May Accident Location: Rig 101 -Rima for Abraj Energy Srvs LLC   WHAT HAPPENED: Floorman working for Abraj Energy Services LLC, a subcontractor of Petrogas was running in drill collars when he pushed the tongs towards the drill collar without holding them by the handles. The jaw made contact with the drill collars and bounced backwards pinching his finger. Handle which should have been used Pinch point which crushed finger • RESULTS: • Broken Finger. • 14 Work Days Lost.

Back to Main Page LTI Notification LTI No 17 2012 25th June Accident Location: Al Burj in Nimr WHAT HAPPENED: A Shaleem Petroleum Trainee Roustabout was lifting the pump barrel from the catwalk to pump shop using a forklift. While trying to pull the pump barrel inside the container, the load rolled and dropped on his foot. • RESULTS: • Broken Foot. • 56 Work Days Lost.

Back to Main Page LTI Notification LTI No 18 2012 2nd July Accident Location: Rig59 - Haban WHAT HAPPENED: Dalma Energy Mechanic and Derricman were adjusting the vertical position of the stabbing board in the derrick when the cable connecting the stabbing board and counter weight came off the pulley. The air winch was used to put back the pulley. The counterweight dropped and hit the derricman’s hand breaking his finger. • RESULTS: • Broken Finger. • 42 Work Days Lost.

Back to Main Page LTI Notification LTI No 19 2012 5th July Accident Location: Rig 4 – Anzouz – In Bahja Area WHAT HAPPENED: A Weatherford Crane Operator was working on a crane when he heard a strange sound coming from the engine. On investigating, he opened the radiator cap. He got 2nd degree burns when hot water splashed on his hand, forearm and neck. • RESULTS: • 2nd Degree Burns. • Several Weeks Lost.

He broke his finger when it got trapped between the stopper on the ladder and the rung. Pinch point caused by stopper on the upper section of the ladder • RESULTS: • Broken Finger • 10 Work Days Lost.

Back to Main Page LTI Notification LTI No 21 2012 6th July Accident Location: Baker Hughes – Qarn Alam Yard WHAT HAPPENED: A Baker Hughes Artificial Lift Technician was checking the motor serial numbers and moving them from an old storage rack to a new steel support rack. He broke his foot when the motor slid off the rack and fell onto his foot. Old rack the motors were resting on Motor rolled and slid off the side of the rack • RESULTS: • Broken Foot • 42 Work Days Lost.

Back to Main Page LTI Notification LTI No 22 2012 17th July Accident Location: Kholoud - Fahud WHAT HAPPENED: A Driver’s Helper working for Al Katheery broke his leg when the sewage vacuum tanker he was travelling in was involved in a rollover incident. The tanker was travelling to Kholoud transporting sewage from Rig-78. RTA • RESULTS: • Broken Leg • 42 Work Days Lost.

Back to Main Page LTI Notification LTI No 23 2012 2nd August Accident Location: Polymer Projects Site - Nimr WHAT HAPPENED: Two Gulf Petroleum Services (GPS), plant mechanics were replacing the old teeth of an excavator bucket with a new one. During replacement, one worker was hammering on the old teeth for removal while his co-worker (IP) was standing approximately 1.5 meter away. A piece of small metal from the old teeth flew off and penetrated into the IP’s abdomen, resulting in a bleeding wound. • RESULTS: • Metal Penetration into abdomen • 14 Work Days Lost.

Back to Main Page LTI Notification LTI No 24 2012 13th August Accident Location: Pipeline Pigging- Marmul WHAT HAPPENED: Al Turki Pipeline Technician twisted his knee when he fell on the pipeline as the crew were doing pigging job. The pig got stuck. They started banging the pig to get it released. As it was released it jerked and produced a loud noise causing the IP to tip over. Knee injured when falling over piping • RESULTS: • Twisted Knee • 28 Work Days Lost.

Back to Main Page LTI Notification LTI No 25 2012 16th August Accident Location: Rig 78 KCA Deutag - Rabiha Fahud WHAT HAPPENED: KCA Deutag Floor man crushed his thumb and finger when his hand was trapped between the chain and block while the rig crew were laying down the top drive. • RESULTS: • Finger and Thumb • 28 Work Days Lost. Pinch point causing the injury

Back to Main Page LTI Notification LTI No 26 2012 2nd September Accident Location: Casing Yard - Fahud WHAT HAPPENED: Sanna Desert in Fahud. crew were in the casing yard re-arranging casing pipes on a storage rack to prepare them for a forklift to transfer them over to an inspection rack. There were two pipes remaining on the second level of casings which were resting on wooden dunnage (wooden beams) to separate them from the level below.  As the team manually rolled the pipes along the dunnage and the second pipe approached one end, the other end flipped into the air and struck the IP, fracturing his jaw.   • RESULTS: • Broken Jaw • 14 Work Days Lost.

Back to Main Page LTI Notification LTI No 27 2012 5th September Accident Location: Rig 7 - Marmul WHAT HAPPENED: Sea and Land Assistant Driller placed the air operated slip assembly in the bowl of the spider for inspection. The latching device holding the slips was not latched in position.  He was closing the door of the spider with his left hand, when the latching device dropped down on to the bowl of the spider and trapped his right hand fingers between the latching device and top of the bowl crushing the ring finger tip. • RESULTS: • Finger Tip Amputation • 5 Work Days Lost.

Back to Main Page LTI Notification LTI No 29 2012 17th September Accident Location: PDO Electrical Workshop - Fahud WHAT HAPPENED: PDO Electrical Technician (IP) was carrying out repair works for an HV Circuit Breaker (CB) in the workshop.  He injured his finger when it was trapped between the rotating shaft and the CB body causing. • RESULTS: • Finger Injuries • 28 Work Days Lost.

Back to Main Page LTI Notification LTI No 30 2012 20th September Accident Location: Fatality at Amin Steam Pilot Project – Nimr Area WHAT HAPPENED: ATE (Al Turki) mechanical crew were using a 25 ton crane to lift pipe sleeves.  During the lifting operation, the deceased went to fetch slings from the crane’s tool box located above the cranes battery.  The contents of the box could not be accessed from ground level so he stood on the outrigger to reached over.  He placed himself in the arc path taken by the cranes counter weight which rotated and crushed him against the toolbox. He immediately fell unconscious and was taken to STS camp clinic in Amal where he was given CPR but could not revived, thus pronounced dead. Back to Main Page LTI Notification LTI No 31 2012 20th September Accident Location: Main Pumping Station - Fahud WHAT HAPPENED: STST employee broke his arm while working near to the main line pumping station when he felt dizziness. He lost his balance and fell into a trench landing on his hand. 0.7m trench • RESULTS: • Broken Arm • 28 Work Days Lost.

Back to Main Page LTI Notification LTI No 32 2012 20th August Accident Location: Dhulaima – Malan Road - Lekhwair WHAT HAPPENED: Al Katheery skip Truck driver broke his leg when his truck collided with a military light vehicle.   Three army personnel were killed and two of their personnel had Lost Time Injuries in this incident. RTA • RESULTS: PDO • Broken Leg • 14 Work Days Lost. • RESULTS: Non PDO Work Related: • 03 Non Work Related Fatalities • 02 None Work Related Injuries
