  8. What does compliance mean? • Fully immunized = vaccinated according to national recommended immunization schedule • Immunity by lab test • History of chickenpox • Exemption on file Washington State Department of Health

  11. Immunization Requirements - DTaP • 5 doses required for K-12th grade Exception: 4 doses required if 4th dose given >4th birthday Washington State Department of Health

  12. Immunization Requirements - TdaP • 1 dose required for 6-12th grade IF student is 11 years of age or older Exception: Tdap given between 7-10 years of age counts for the Tdap requirement Washington State Department of Health

  13. Immunization Requirements – DTaP/TdaP/Td • DTaP: 6 years of age and younger • Tdap and Td: 7 years of age and older Washington State Department of Health

  14. Immunization Requirements – DTaP/TdaP/Td Students 7 years of age or older

  15. Immunization Requirements – Hepatitis B • 3 doses required for K-12th grade • K-7th grade: Dose 3 must be given min age of 24 weeks • 8th-12th grade: Dose 3 must be given min age of 4 months Exception: Students 11-15 years of age can get 2 doses of adult Recombivaxhep B vaccine

  19. Immunization Requirements - Varicella • 2 doses required for K-6th grade • Healthcare provider verification of varicella disease required; no parent verification Healthcare provider verification of chickenpox disease

  29. Completing the Annual Report: How to Complete • Does your school have • Kindergarten? • 6th grade? • Both? • No – Your report is complete for the year. If using our immunization system, you select the Save and Submit button. • Yes – CONTINUE to Part C Washington State Department of Health

  32. How to Report • Option 1. Report online using the • Option 2.
