Methods • 11 Dutch rehabilitation centers • Nutritional status determined in 366 inpatients • Severely undernourished:BMI<18.5 (≤65y) or <20 (>65y) and/or unintentional weight loss ≥5% in the past month or ≥10% in the past six months; • Moderately undernourished:BMI 20- 22 (>65y) or 5-10% unintentional weight loss in the past six months; • Not undernourished • Diagnostic accuracy was measured for SNAQ, SNAQRC, SNAQ65+, MUST and MNA-sf. Results • 28% severely undernourished • 10% moderately undernourished • Within 82%of the patients, undernutrition was caused by unintentional weight loss • In the undernourished group, 28% was overweight (BMI 25-30) and 19% was obese (BMI>30) • The SNAQ65+ showed the best diagnostic accuracy (Se 96%, Sp 77%, PPV 62%, NPV 98%) • The MNA-sf scored very low, with a specificity of 44%. Discussion The prevalence of undernutrition in Dutch rehabilitation centers was high (28%). High percentages of overweight and obese patients were found within the undernourished group. Therefore screening on undernutrition alone is not appropriate. We advise rehabilitation centers to determine the BMI and the percentage of unintentional weight loss as a basic patient characteristic in all patients.
