  5. What’s New for 2013 • Supplemental Life • Elect 1 to 8 times basic annual earnings, up to $1,000,000 in coverage • Reduced Cost

  6. What’s New for 2013 • Supplemental Life – First Time Election • Elect 1 to 6 times basic annual earnings • Guaranteed Issue - $500,000 • Enroll at www.Prudential.com/webenroll

  7. What’s New for 2013 • Spousal Life • Same coverage level • Cost reduced for most staff • Child Life • $5,000 increments to $20,000 maximum coverage • $.03 per $1,000 of coverage

Annual Enrollment Checklist • October 1st – October 15th- Enroll/Change Online • Life Insurance Enrollment – www.prudential.com/webenroll • Spousal Surcharge Waiver Form, if applicable • Medical Rate Assistance Program, if applicable • Adult Child Eligibility Verification Form – complete for each individual age 19 and over enrolled in medical plan • Family Member Addition Form – complete if adding anyone new to your benefits
