  3. CONCIERGE/ LAY NAVIGATOR • Meet and greet • Facilitate transportation for appointments • Refer to ACS Resource staff • Refer to Survivorship Navigator • Refer to Nurse Navigator • Provide education materials • Oversee community events in our building

  4. Survivorship Navigator • Definition of survivorship • LCSW • Financial assistance • Insurance • Supportive counseling • Cover our inpatients, outpatient radiation and medical oncology group • Survivorship support group • Distress Tool

Nurse Navigator • Oncology Certified Nurse • Provides support, education, advocacy and guidance to all types of cancer patients in inpatient and outpatient settings • Early involvement with new diagnosis • Facilitate appointments • Liaison with VNA • Support with transition of care • Pulmonary nodule tracking • Survivorship Care Plans

  6. Cancer Registrars • You play a critical role - thank you! • Close communication with registrars • Pathology reports • Early involvement with patients • Facilitate appointments, especially with primary care physicians • Tumor Boards • COC Visit

  7. Pulmonary Nodules • Fleischner Recommendations • CT to Biopsy • Positive biopsy to Pulmonary appointment • Arrange metastatic work-up • Virtual Clinic with pulmonary, medical oncology, radiation oncology and thoracic surgeon

  8. SURVIVORSHIP CARE PLANS • Journey Forward • Breast cancer patients • 1 month post completion of treatment • Copies to all physicians • Meet with nurse navigator and survivorship navigator

  9. Benefits of Navigators • Patient and family satisfaction • Retention of patients in system • Physicians are happy with our supports to their patients • With social worker involved, starting or renewing insurance, facilities get reimbursed

  10. Observations • Information given to patients and families needs to be accurate and timely provided by the right person • Be consistent, available and flexible • Be a good listener
