Goal of the Transplant Growth and Management Collaborative (TGMC) • “Save or enhance thousands of lives a year by maximizing the number of organs transplanted from each and every donor and building the necessary capacity within the Nation’s transplant programs to transplant 35,000 organs annually.”

  5. Close “the Gap”; Reach “Capacity,” Achieve 35,000. • Capacity is reached when a DSA produces 75% conversion rates; 3.75 organs transplanted per donor and 10% DCD (with 2.75 OTPD) and generates sufficient resources to transplant the resulting organ supply.

  6. Results of Michigan Transplant Centers’ PDSAs • A LEAN “no show” process was created that decreased the patient “no show” rate from 30% to 19% in 2 months. • A children’s hospital has implemented an approach team. • A kidney program has added a transplant surgeon and a list management coordinator. • A thoracic program has added 2 transplant surgeons. • A liver program has added a transplant surgeon.

  7. Results of Michigan Transplant Centers’ PDSAs • Daily e-reports reflect every vented patient for virtual rounding. • Length of time required to prepare a patient for placement on the transplant waiting list has been shortened. • Two transplant centers have dedicated a hospital FTE to support and facilitate organ donation activities.

Objectives for the NLC • Celebrate and accelerate successes, recognize contributions of the HHS Medal of Honor winners. • Quicken spread of best practices to 950 hospitals that represent 90% of U.S. donor potential. • Close the Organ Transplantation Growth & Management Collaborative. • Sustain national accountability of all three estates for achieving/exceeding national goals (75%, 3.75) with the intent to increase the number of patients benefiting from a transplant. • Increase the momentum, recommit to an ongoing collaborative process, and provide insights into specific, experience based practices.

Thursday, 10/23, Learning Session Day 1 • Collaborative successes and what we are learning. • Mini-plenary for each of 3 tracks. • “Compassionate End of Life Care” track (EOL) • “Stewardship of the Gift of Life” track (STW) • “Building the Capacity to Transplant 35K deceased donor organs” track (CAP) • Breakouts in 3 tracks. • 2 cross-cutting topics • Pediatrics- and DCD-related presentations • Closing joint strategic session.

Friday, 10/24, Learning Session Day 2 • Mini-plenary for each of 3 tracks. • EOL: Consent-related topics • STW: Maintaining high conversion rates • CAP: Setting OTPD for pediatric donors • Breakouts in all 3 tracks. • Strategy session and town hall meeting. • Learning Congress adjourns at 4 pm. • HHS Awards celebration and dinner in the evening.

  12. Medal of Honor Ceremony • 369+ hospitals will be recognized for achieving 75% goal. • 72 hospitals will receive additional recognition for achieving BOTH 75% and 3.75 goals. • Dress: Black Tie optional.
