  10. Infección por virus BK • Confirmación del diagnóstico virológico 1. Inmunohistoquímica - SV40 - Específico para BK 2. PCR específica 3. Microscopía electrónica

  11. Infección por virus BK • Diagnóstico sin confirmación histológica - Clínica - Descartar rechazo u otra causa de disfunción - Orina muy positiva - PCR cuantitativa en sangre Nickeleit V, et al. N Engl J Med 342:1309, 2000

  12. Infección por virus BK • HECHO: • Enfermedad sólo en inmunocomprometidos • No hay presentación clínica específica • Diagnóstico de confirmación: sólo histológico

Infección por virus BK • Importancia del estudio citológico de orina • Como tamizaje: Todos los casos con nefritis intersticial tienen abundantes células con inclusiones en orina - Si la orina es persistentemente negativa puede descartarse la enfermedad Arias LF, et al. Hum Pathol 30:970, 1999 • Seguimiento luego del diagnóstico - Si hay respuesta al tratamiento la cantidad de células infectadas debe disminuir Arias LF, et al. Acta Cytol 47: 1145, 2003

  15. Infección por virus BK • Tratamiento • Disminución de la inmunosupresión Rechazo • Cambio de inmunosupresores • Antivirales: Cidofovir

  16. Infección por virus BK • Seguimiento - Clínico - Citología de orina - Carga viral: PCR cuantitativa en sangre
