  4. The Old Paradigm • The “acute care” model • Symptom-driven • Disease well-established • Opportunity for prevention is lost • Contact initiated by patient • Often unaware of his/her own health status • Not knowledgeable about the disease

The Old Paradigm • Medical care is delivered on a case-by-case basis • Cannot determine which conditions are the most problematic or where the process has failed • Cannot assess demand for services or allocate resources • Cannot prioritize patients based upon likelihood of benefit • Cannot identify and disseminate “best practices”

The Old Paradigm • Visit-based care • Most difficult to coordinate • Most expensive option • Most vulnerable to access barriers (geographic, economic, and social) • Misses people who do not attend clinic (often the most needy) • Treatment tied to the availability of appointments – not response to the intervention • Long delay between when findings are “discoverable” and discovered • The “one-size fits all” appointment never fits anybody • Too soon for some problems and too late for others • Too short for some patients and too long for others

  7. The Old Paradigm • Physician-centric • Creates “bottleneck” in workflow • Physician often assigned menial tasks while team members are under-utilized. • Team disabled in physician’s absence

PCMH • The Patient-Centered Medical Home • PROMISES: • Team based care, population management, complicated case management, improved access via better utilization of team members (top of your license work) • SHORTCOMINGS: • Often remains physician-centered and physician-dependent • Top of your license work requires coaching and support that may or may not be available when needed • There is still no fail-safe mechanism to prevent “falling through the cracks”
