  2. What is the NEW Devon Clinical Commissioning Group? • Largest CCG in the country • Three localities • Population 894,900 • Mixed urban and rural populations • 2,330 square miles • Budget for 2012/13 - £1.1 billion

What’s NEW since we became a CCG: Local GP perspective • It is a GP and clinician led organisation • GP Practices are members of the organisation and elect a representative to sit on the locality board and governing body board. • Clinicians are having a real influence in the decision making of the health services that are provided • Feeling empowered to shape the local health services in the local area in collaboration with view and opinions of the public.

  4. The Eastern Locality – Who we are and what we do The Eastern Locality is part of the Northern, Eastern and Western (NEW) Devon CCG. It has a population of c380,000 (expected is to rise by another 11.9% by 2026). It is represented by four sub localities: Exeter, Mid Devon, WEB (Woodbury, Exmouth and Budleigh) and Wakley (Axminster, Seaton, Sidmouth, Honiton and Ottery St Mary). Sub localities are smaller populations within Eastern Devon where care is commissioned uniquely to ensure it meets their different needs.

Transforming Community Services (TCS) Focus on services that take place in people’s homes and other community settings such as clinics and community hospitals • Develop community services strategy, principles and outcomes – ‘what’ should services focus on and be like in the future • Achieve sustainable delivery arrangements for community services – ‘how’ will services be configured or arranged and organised. A wide range of engagement activities • More than 20 local health summits across the county • Voluntary and 3rd sector events • Discussions with Devon’s Joint Engagement Board • Surveys and events led by the local Healthwatch teams • Reference group discussions with a range of key stakeholders • Discussions at Scrutiny Committees • Discussions at Health and Wellbeing Board • Engagement of District Councils (meetings or local events) • Involving clinicians to bring their experience to the programme • Range of locality communications such as web and newsletters • Reviews of prior engagement and feedback received

  10. Where does the better care fund fit in? The £3.8bn Better Care Fund (formerly the Integration Transformation Fund) was announced by the Government in the June 2013 spending round, to ensure a transformation in integrated health and social care. The Better Care Fund (BCF) redirects existing funds into a single pooled budget to support health and social care services to work more closely together in local areas. The better care fund sets out national conditions and metrics to be achieved when it is in place from April 2015/16 onwards

Better Care Fund (BCF) Announced in June as part of the 2013 Spending Round (initially called the Integration Transformation Fund) Primary concern is transforming local services so people are provided with better integrated care and support 2014/15 Further £200m transferred from NHS to Adult Social Care, in addition to the £900m already announced What is the BCF? 2015/16 £3.8bn to be deployed locally on health and social care through pooled budget arrangements Payment for performance element National conditions £1.9bn based on existing funding £1.9bn of NHS funding Key performance metrics

BCF – Key Features • April 2015 • Payment for progress against 4 of the national conditions: • Protection of ASC • 7 day services • Agreement re impact on acute • Accountable lead professional • Progress against local metric and DTOC and Avoidable admissions 14 Feb 2014 Submit draft version of plan 4 April 2014 Submit revised version of plan October 2015 Payment for progress against all of the national and local metrics April 2014 (assumed) Payment of 2014/15 £200m to progress BCF (conditional on joint sign-off of BCF)

Better care fund funding over time • Focus is on greater integration • This is not new money – we are already using these funds elsewhere. • From 2015 / 2016 there will be progress towards national conditions: • Protecting social care services • 7 day working • Data sharing • Joint assessment • Evidence based outcomes • Integration vision

  14. How can you influence the consultation document? • Create a map of market provision across the locality • What do you do? • Where do you do it? • What is your capacity? • How do we measure quality? • Can you help with the metrics discussed? Service user feedback? • What are the outcomes of your service? • Do you cater for any specific health needs i.e. Dementia? • Are you interested in development of health hubs? How could you contribute to this service?
