What is RPMS iCare? • iCare is a Population Management tool • iCare provides an easy way for providers to identify and manage their patients proactively • iCare is an interface to underlying RPMS data • iCare provides the framework and the background processes to enable this functionality • iCare utilizes existing RPMS applications and logic
