The process for manually adding a Plan of Care (PoC) has undergone changes to enhance usability and align with Meaningful Use requirements. To add a problem, click the “+” sign, select the “Problem Set” radial button, and type "Nursing" in the text area before clicking the search button. Expand the list of Plan of Care Problems and select the relevant issue to proceed. These updates help nursing identify relationships between PoC Problems and physician-driven problems, ensuring a clear distinction between chronic and active issues across accounts.

Manually adding a Plan of Care process has changed a little bit!

Step by Step Directions to manually add PoC You must Click the + sign to add a problem as noted on previous screen shot Then you must select the “Problem Set” radial button Type Nursing in the free text area Click Search button Then open the Plan of Care Problems by clicking on the triangle open/expand list
Once you select the problem the process returns to normal to add the plan and interventions.
Why the Changes? Ultimately these changes are required for Meaningful Use. These changes will assist nursing in determining whether the problems are related to the PoC Problems verses the physician driven problem list Problems (diagnosis) that they will need to be placing in the very near future from the Clinical summary screen. Problems that are chronic will be continued from account to account where active problems will be resolved and removed from the problem list.
