  4. Step II: Print out & complete form Completed when you print the form LEAVE BLANK – We sign & supply photo

  5. Bottom half of form Check one Sign & date DO NOT MAIL!

  6. Step III: Bring form to Sharon McChesney, VH 107P • Step IV: UASOM certifies & mails NOTE: Per USMLE, may take 4-5 weeks from receipt of form there before you can choose a date
