General Information • The LA plan is similar to a grant application. Once it is approved, separate written or verbal approval for activities or purchases in the plan will not be needed, unless the plan is modified. • The purpose behind using the specific forms & tools in this document is to have a uniform method of evaluating all LAP's submitted. • The agency should use the completed Technical Assistance Packet & applicable reports or data to prepare the plan. General Information • Complete all parts carefully & accurately. Any plan received lacking any part of the required information will be returned for completion. Approval of the WIC LAP will not proceed until it is complete. • Make sure you start by using the final version of your 2007 LAP that was returned by the state with our approvals on it, that you updated with your mid-year evaluation of your Goals and Objectives. • Do a “Save As” of your current LAP with your Mid-year evaluation saving it as AgencyName LAP 2008.xls. General Information • If you cannot find your LAP document, we can send you the approved one from last year (contact Glenn.Studebaker@dhss.mo.gov if you need it replaced). General Information • Reminder: In 2007 we began to require and recommend evidence-based interventions in: • Breastfeeding (required) • Physical Activity/Fit WIC (recommended) • Caseload (recommended) • Cost Per Participant is increasing to $9.50 • Caseload continues to rise statewide.
