  2. HWICE 895-01Management Assessment - Part I The 895-01 is the 7 page Annual Management Assessment Part I Data is from the State FY July – June. This report runs the beginning of July, and is mailed, if you do not receive it by the 16th of July. Call your HPR III. At Mid-year you want to look at the Semi-annual version of the report, the 897-01. Missouri Department of Health and Senior Services

  5. HWICE 895-01Management Assessment - Part I The first thing to check on the 895-01 is selected Risk Factors WIC Risk Factors changed in June 2007 • from 2 digit Alphanumeric Codes • to 3 digit Numeric Codes used by USDA Missouri Department of Health and Senior Services

  6. HWICE 895-01Management Assessment - Part I In some cases Risk Factors just changed their names. (Prematurity was X5 now is 142) In other cases old risk factors were combined to create a new one. (Anemia was B1, Y1 and Y2 (High Risk) are now combined into 201) Some are new (Short Stature or at Risk of Short Stature 121) Still others have new definitions that will impact how many participants receive the Risk Factor (Failure to meet Dietary Guidelines D1 is now 401) Missouri Department of Health and Senior Services
