Originally completed by Catalan & Clark 2008 • Audit Dates • Jan 2012 • Jan 2013 • Due Jan 2014

Safety Audit Questions • Training courses attended in the last 12 months • Breakaway training • Interviewing potentially violent patients • Recognition and prevention of violence • Using de escalation techniques • Understanding of local safety policy and procedures • Reporting of adverse incidents • Safety induction • Personal perception of safety • Incidents and outcomes

Recommendations • This audit results will be shared with all trainees and educational supervisors • Safety audit proforma will be sent to all trainees and educational supervisors and will be discussed routinely in three way meetings conducted in every placement • Safety to be discussed as part of induction to the scheme, trust and placement • Safety to be discussed routinely in supervision with trainee/educational supervisor • Safety audit will be conducted in 12 month’s time • Safety will remain a regular agenda item on the STSC meeting • Safety updates on every annual trainer/trainee away sessions
