Models to date largely unstructured • Allows for grading of importance of goals • Very little potential for aggregation • Use of more structured care plans can be personalised • Use of a more structured approach gives potential for interoperability and reporting • Builds towards the care.data requirements

The absence of a declared owner or sponsor for this product means that we are proposing to withdraw it and are keen to hear views and understand the impact of not maintaining this content and its associated services • We regard this as an opportunity to maximise engagement and collaborative working at and across all levels and functions • We have a number of questions to determine how Information Standards could best support the development of a standard for care planning • In addition to the formal consultation about the proposed withdrawal a discussion is also being established that can be found via www.infostandards.org/careplanning

  9. This consultation commences via Citizen Space on 13 November 2013 link via www.infostandards.org • This consultation will end on 8 January 2014 • An analysis response report will published week commencing 20 January 2014
