Who We Are Kelowna General Hospital • A 16 bed medical/renal acute care ward in Kelowna BC • Most beds have bed alarms • Staffing includes: 2 RNs and 2 LPNs on days and 2 RNs and 1 LPN after 3pm • For the purposes of data collection, we used 8 beds on Team 2
AIM • 40% reduction in fall rate from baseline • 40% reduction in fall related injuries • 100% completion of a Fall Risk Assessment Tool (FRAT) upon admission • 100% completion of FRAT following a fall or significant change in medical status • 100% of “At Risk” patients with a documented fall prevention / injury reduction plan
