  2. Who We Are Location of Facility Number and type of Patients/Residents/Clients: 2 Geriatric Mental Health Inpatient Units. Each unit has 20 beds. Clients are 60 years and older living with a mental illness and or addiction issues.

  4. AIM To reduce injury from falls by 40% from baseline by March, 2011. Increase the amount of clients identified “at risk” for falls to have a documented falls prevention intervention strategy (100%) by March 2011. Increase the amount of clients screened using a “falls risk assessment form” upon admission (100%) by March, 2011.
