Training Objectives • Configure RPMS & EHR to provide patients with an electronic copy of their discharge instructions upon request. • Review newly created TIU note titles that document requests for electronic copy of discharge instructions and fulfillment of the requests. Threshold More than 50% of all patients who are discharged from an eligible hospital or CAH inpatient or emergency department (POS 21 or 23) and who request an electronic copy of their discharge instructions during the EHR reporting period are provided it. RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: requested an electronic copy of discharge instructions during the EHR reporting period HAVING: a hospitalization during the EHR reporting period (defined as Service Category of H) WHERE: the Discharge Type was Regular Discharge, Transferred, or Irregular Discharge OR HAVING: an emergency department visit during the EHR reporting period (defined as clinic code of Emergency Department-30 and Service Category of A) Electronic Copy of Discharge Instructions
