Training Objectives Configure RPMS & EHR to provide patients with an electronic copy of their health information upon request, including: • Diagnostic test results • Problem list • Medication lists • Medication Allergies • Discharge summary & instructions (EH and CAHs) • Procedures (EH and CAHs) Electronic Copy of Health Information

7. Threshold More than 50% of all patients of the provider who request an electronic copy of their health information are provided it within three business days. Electronic Copy of Health Information

RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: at least one face-to-face visit with the provider in the 365 days prior to the end of the EHR reporting period AND HAVING: requested an electronic copy of their health information, (the Patient/Agent Request Type value in the Release of Information (ROI) package is equal to “Electronic”) WHERE: the request for their health information was made at any time from the first day of the EHR reporting period through four business days prior to the end of the EHR reporting period (determined by the Date Request Initiated field in ROI) Electronic Copy of Health Information

RPMS Reporting Logic Numerator Inclusions: COUNT: each patient in the Denominator WHERE: the patient was given an electronic copy of the health information within three business days from the request date (determined by the ROI fields of Record Dissemination where the value is “Electronic,” and Disclosure Date) Electronic Copy of Health Information
