MU Objective and Measure Provide patients with an electronic copy of their health information (including diagnostic test results, problem list, medication lists, and medication allergies) upon request. More than 50% of all patients who request an electronic copy of their health information are provided it within three business days. Electronic Copy of Health Information

RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: at least one face-to-face visit with the provider in the 365 days prior to the end of the EHR reporting period AND HAVING: requested an electronic copy of their health information, (the Patient/Agent Request Type value in the Release of Information (ROI) package is equal to “Electronic”) WHERE: the request for their health information was made at any time from the first day of the EHR reporting period through four business days prior to the end of the EHR reporting period (determined by the Date Request Initiated field in ROI) Electronic Copy of Health Information

RPMS Reporting Logic Numerator Inclusions: COUNT: each patient in the Denominator WHERE: the patient was given an electronic copy of the health information within three business days from the request date (determined by the ROI fields of Record Dissemination where the value is “Electronic,” and Disclosure Date) Electronic Copy of Health Information

7. RPMS Reporting Logic Measure Exclusion: • EPs who have no patients in the denominator are excluded. • Eligible hospitals and CAHs who have no patients in the denominator are excluded. Electronic Copy of Health Information

8. What HIM Needs to Know • If requested, an electronic copy must be provided within 3 business days of the visit • A record of the request and release must be recorded in ROI pursuant to the Privacy Act & the HIPAA Privacy Rule Electronic Copy of Health Information
