  3. 1. 2010-2013 IIPP Priorities • Increase Screening and Coverage at: • Juvenile Detention Centers • School Based Clinics • Family Planning Clinics and • Other settings serving high risk populations • Reduce screening in low risk women/sites with less than 3.0 % CT positivity rate • Improve QA monitoring: Utilize a self assessment tool for FP and STD programs

  4. 1. 2010-2013 IIPP Priorities Cont. • Reduce GC and the high rate of health disparities • Foster increased CT/GC in private sector by collaborating with HMOs • Maintain/improve data reporting timeliness and completion rates and timely feedback to IIPP screening sites • Expedited partner therapy • Encourage use of EPT for partner management

School Based Health Center Initiative • IDHS Bureau of Community Nursing & IDPH STD Section • Objective: increasing CT screening at SBHC • Developed a PowerPoint presentation w/ site specific data • To date 27 of 41 (66%) teleconferences have occurred with SBHC, IDHS, IDPH • Data: 2008 compared to 2009 • Ct screening increased from 6,313 to 7,383 (17%) • 39 (5%) additional CT cases identified • 20 (11%) additional GC cases identified

School Based Health Center Initiative • CDPH/CPS/IDPH/SBHC - screen HS juniors and seniors in select Public Schools in the 2009-2010 academic year (February through May 2010) • Goal: reduce sexually transmitted infections among teens and adolescents in Chicago • 11th and 12th grade students participate in this opt-out education and screening project

School Based Health Center Initiative • Students receive a 30-minute STI health education presentation and a packet: • one page overview of the project, • instructions • consent forms • specimen form • information on STIs • information on other public health services • a urine specimen cup and • a brown paper bag for urine specimen • Students who want to test complete consent and obtain urine • Results will be shared at next RVIPP meeting

IIPP FP & STD Clinic Self Assessment • Title X FP Clinic - Self Assessment Findings • Response Rate: 42 out of 44 (96%) clinics • 42 of 42 (100%) indicated they conduct a sexual history • 42 of 42 (100%) screen all sexually active females under 26 years annually • 41 of 42 (98%) screen clients over 26 years only if they have a risk factor

IIPP FP & STD Clinic Self Assessment • Title X FP Clinic - Self Assessment Findings • 38 of 42 (91%) notify and refer infected/untreated clients w/in 3 days for Tx • 39 of 42 (93%) schedule rescreen 3 to 4 months after treatment • 39 of 42 (93%) provide clients with chlamydia information • 42 of 42 (100%) provide partner counseling to each client infected with CT/GC • Chart Audit: 978 out of 1,015 (96%) of patient charts followed the IIPP Selective Screening Criteria.

IIPP FP & STD Clinic Self Assessment • STD Clinic - Self Assessment Findings • Response Rate: 28 out of 29 (97%) clinics • 27 of 27 (100%) indicated they conduct a sexual history • 21 of 27 (78%) screen all sexually active females under 26 years annually • 16 of 27 (59%) screen clients over 26 years only if they have a risk factor

IIPP FP & STD Clinic Self Assessment • STD Clinic - Self Assessment Findings • 23 of 27 (85%) notify and refer infected/untreated clients w/in 3 days for Tx • 17 of 27 (63%) schedule rescreen 3 to 4 months after treatment • 24 of 27 (89%) provide clients with chlamydia information • 25 of 27 (93%) provide partner counseling to each client infected with CT/GC • Chart Audit: 405 out of 439 (92%) of patient charts followed the IIPP Selective Screening Criteria.

IDPH Laboratory Update • Lab Information Management System • StarLims: STD data component is a priority • Implementation date is February 2010 • Pooling of CT/GC Specimens • After the implementation of StarLims • Validating oral and rectal specimens: • proposal written/reviewed for the validation study • clinic contacted to assist with the study • CDC contacted- developing a validation panel for the BD ProbeTec assays

Cook County Jail 2009* Chlamydia Screening • Routine Screening During Intake • Females: 1,371 Tests (6.6% Positivity) • Males: 254 Tests (26.8% Positivity) • 160 Infections Identified • Boot Camp • Males Only: 550 Tests (9.8% Positivity) • 54 Infections Identified • Male Screening • 1,532 Tests (11.4% Positivity) • 175 Infections Identified Not yet complete 2009 data

  17. 7. Expedited Partner Therapy • Became law Jan 1, 2010 • Guidelines (collaboration with CA and MN) • Patient Information Sheets-English/Spanish • Documents can be found at: • www.idph.state.il.us • Encouraging sites to utilize EPT as an additional partner management tool

  18. 8. Chlamydia Evaluation Initiative • One of eight sites funded by CDC • Partners: • Local health department • IDPH • HMO • Two Local Hospitals • FQHC • University Health Center

Chlamydia Evaluation Initiative • Objectives • Determine to what extent clinicians are following screening guidelines & amount of chlamydia screening occurring at the various settings • Increase clinician awareness of importance of screening and ultimately increase screening in the correct populations • Provider survey • Chart audits
