Successes and Challenges in Diabetes Management in Aberdeen City

Dr. Alasdair Jamieson, Clinical Lead for the City Community Health Partnership, highlights key successes and ongoing challenges in diabetes care during 2008/2009. Successes include a 17.3% increase in detection rates and the implementation of combined retinal and foot screening programs. However, challenges remain for 2009/2010, such as achieving practice-based care goals over 65% and enhancing diabetes education. New Quality and Outcomes Framework (QOF) targets for HbA1C levels pose further demands on healthcare providers while addressing the risk of over-medicalization.

  2. SUCCESSES 2008/2009 Increased Detection (up 17.3% from 2007) Increase Practice Only Care ( now 59.9%) Improved Practice Only Care Equality ( no practice seeing less than 25%)

  4. CHALLENGES 2009/10 • Continue move to Practice based care(65% plus) • Look to expand Practice based teams/Diabetes education • Skills /knowledge to convert Type 2 diabetics to insulin within primary care

  5. CHALLENGES 2009/10 • NEW Q.O.F TARGETS !!!!! • HbA1C 7 or under > 50% • HbA1C 8 or under > 70% • HbA1C 9 or under > 90% Reduction in morbidity / mortality from diabetes OR • Risks of over-medicalisation, iatrogenic illness and demotivated staff
