FVHCA Requirements • School coordinators must ensure all FVHCA.org requirements are met prior to starting a placement experience with us. • FVHCA Health Requirements • Including FVHCA Hepatitis B Documentation Form • Background Check Information • Including an annual Background Disclosure Form (BID) • View FVHCA Orientation Modules • Sign FVHCA Forms A, B, & C All FVHCA Requirements & documentation should be housed on the school end & available upon request. School coordinators send the FVHCA Required Information Form (RIF) for Undergraduates or Graduate Placement Form, when all FVHCA requirements are met.

Orientation Checklist Before you start your clinical or student experience at our sites, Print the appropriate orientation checklist Aurora Undergraduate (Hospital) Student Orientation Checklist Aurora Undergraduate (Clinic) Student Orientation Checklist Aurora Graduate Student Orientation Checklist 1. Bring to your first day of clinical/experience 2. Review with your instructor or preceptor 3. Sign and Return to Barb Bloch in the AMCO Education Department via interoffice mail

  15. Before you start • Our patients may not recognize the difference between our regular caregivers and students • Before starting review the following policy: Caregiver Appearance Policy • This policy tells you what you can or cannot wear, grooming, fingernails, piercings, tattoos, etc.

  16. Before you start • Aurora is a smoke free organization • Smoking is not permitted in buildings and/or on the facility property • Those that choose to smoke must leave the campus or the property

  17. Before you start • There are no firearms, handguns, or weapons • allowed in any of Aurora facilities • Must be locked in car • If firearm or handgun found, security is notified – • they will keep locked in secure location • (if security is on premises)

Before you start Use of Personal devices during Work Time Team Members & Students with a personal cellular phone, iPad, PDA or other mobile electronic equipment must follow department guidelines in utilization of these devices during work time. Most patient care areas will prohibit the use of devices and require they be kept in a separate area (break room, locker room, etc.…) Team Members & Students should not be using Twitter or posting status updates on LinkedIn, FaceBook, MySpace or personal websites during clinical time.

Badges All students & instructors must be identified with a badge Aurora requires all students and instructors wear a school issued ID Undergraduate Students (and Graduate Students without a school issued ID) must also wear an Aurora student badge. These can be picked up at the HR department, located on the 2nd floor of the main hospital, between the hours of 8:30-4 pm OR by calling (920) 456-7238 Faculty and SRNA badges can be picked up in the Security Department, located on the main hospital floor OR by calling (920) 456-7020 Kindly return all Aurora badges after your clinical rotation.

Patient Experience Please Remember to AIDET + PROMISE: • Acknowledge the patient by knocking first, saying hello • Introduce yourself and your role as a student • Duration: discuss how long a procedure, an interaction, assessment, a test, or results may take • Explain the purpose of your visit and what you will be doing • Thank the patient for their time and close with “is there anything else I can do or get for you?” • Promise: offers the patient a commitment to excellent care

Patient Experience AIDET + Promise • Introduce —Introduce yourself, your skill set, your professional certification • Duration— Duration of your shift, when will you be back, how long will they sit in their chair, when will they get their am cares, etc. • Explanation — Explain step by step what will happen, how to contact – call light, make sure questions are answered, steps in an activity.
