Historical Review • INPO 08-004, Human Performance Key Performance Indicators • June 2008 • Industry Developed • Established significance criteria (consequence based) for HU Events • Event Rate Performance Indicator • Event Rate = # of events (over last 18 mons.) x 10,000 Total person hrs. worked over last 18 mons.

Historical Review • First Quarter 2009, plants started reporting data in Consolidated Data Entry (CDE) system • Monthly number of “Human Performance Errors” • Quartile ratings calculated by INPO based on monthly and quarterly data • Industry data found under “Additional INPO Indicator Results” on Plant Information Center homepage

Challenges with reported data in 2009 • Some stations reporting “errors” not meeting significance criteria in INPO 08-004 and CDE Data Element Manual, INPO 04-004 • Not being detected and corrected by HU leads • Action needed by HU leads • Know who is updating CDE for your site • Ensure they are using correct site indicator for input data • Validate correct data entered

Challenges with reported data in 2009 • Again – only report data meeting the significance criteria for a station event (site clock reset) as defined in 08-004 • Why not change indicator title to “event” instead of “error” • “Event” term already used by INPO for analysis of industry submitted operation experience reports • Still being evaluated by analysis group at INPO

Human Performance Event Performance Indicators • There is other “Human Performance Events” data in the Plant Information Center (PIC) • Not the same as the new 08-004 indicator • Stations report number quarterly • Determined by INPO Analysis Group review of submitted Operation Experience reports • INPO Analysis Group tags Human Performance as applicable during analysis of submitted Operation Experience reports

What’s next? • 18 months of industry data will be available end of second quarter 2010; then • Calculation will start using rolling 18 months (aligned with 08-004) • Indicator will move into Plant Information Center (PIC) • Targeted for implementation after 2010 second quarter • Quartile values calculated using 18 month rolling data

  11. What’s next? • Industry team worked hard to establish station event criteria in 08-004, but based on roll-out experience it is time to revisit the criteria.

SITE HUMAN PERFORMANCE EVENT-FREE DAY RESET CRITERIA Definition of a Site Human Performance Event-Free Day (EFD) Reset – When an event, as described in the table below, occurs as a result of either of the following: • an initiating action (error) by an individual or group of individuals (Re) (event resulting from an active error) • an initiating action (not an error) by an individual or group of individuals during an activity conducted as planned (Md) (event resulting from a flawed defense or latent organizational weakness created within the last 18 months)
