This study focuses on utilizing Event-Related Functional MRI to map linguistic networks affected by stroke, guiding the application of rTMS for treatment. By evaluating changes in functional networks, we aim to enhance clinical outcomes in stroke patients with aphasia. The stimuli used include Same/Different Judgments, Non-words, Sinewave Combos, and Silence, all presented over headphones and triggered by MRI. The study employs Presentation Event-Related fMRI software with specific parameters detailed, comparing responses to different stimuli types.

Stroke & Aphasia • Stroke is 3rd-leading cause of death • ~700,000 strokes/year in the US • ~ 1/4 have language impairments • Current treatments: • Spontaneous recovery • Speech therapy
Goals • Identify linguistic network functionally. • Target rTMS to this network. • Assess for changes in the functional network, and more importantly, clinical improvement.
Stimuli • 4 types of events: • Same/Different Judgment: • Non-words (VCV - manner, place, voicing) • Notes • Sinewave Combos • Silence • Presented over headphones • Triggered by MRI • Button-box response • Software: Presentation
Event-Related fMRI • TR=8000 Delay in TR=6000 • TE=50 Flip Angle=90 • Slices=27 Thickness=5mm FOV=240mm • # Measurements=52
